1. Managing Dual Practice of Health Workers : Evidence from Indonesia

abstract

Managing dual practice of health workers has often proved to be challenging, especially in emerging countries characterized by weak monitoring and low motivation. This paper exploits an exogenous variation in the initiation of private practice among heads of local public facilities (known as puskesmas) providing primary health care after the introduction of a 1997 health regulation in Indonesia. This regulation required health professionals to apply for a license for private practice at least three years after their graduation. Exploiting the exogenous variation in private practice after the 1997 regulation, we provide estimates of causal effects of dual practice on provision of public health services, distinguishing between the effects when private practice is located at or away from the public hospital. The estimates suggest that dual practitioners (relative to those engaged in public service only) work significantly less hours per week while they see significantly more patients in their public facilities. These adverse effects of dual practice are most pronounced when private practice is held away from the puskesmas. These results have important bearings on human resource management of universal health care provision

4. On the design of equity-oriented pharmaceutical copayments

abstract

In this paper, we propose a new approach to address the problem of designing pharmaceutical copayments. The rationale for positive copayments in our setting lies in the presence of budgetary constraints and, hence, in the need to raise funds in order to finance the costs of the treatments. We use results from the literature on axiomatic bargaining with claims to incorporate criteria of distributive justice into the design of copayments. We find that if the government constrains patient rights to what is medically feasible, equity-based copayments vary from a percentage of the cost of the treatment, to a flat rate per prescription. If the government also takes into account the burden of disease experienced by patients, then copayments based on equity lead to a relation between copayments and clinical status that diverges from those proposals based on efficiency arguments. In particular, we show that equity-based copayments might be increasing in the health gains that the treatments provide to the patients. The reason is that these copayments try to avoid a "double jeopardy" problem, by ensuring that those patients with a large burden of disease do not face also an important monetary cost.

5. Tourist behavior and demand for digital disconnection : a review

abstract

The hyper digitization of the current moment has led to the emergence of new behaviors and trends such as the one that occupies the object of study of this research about vacations free of digital media. The main objective is to shed light on a subject that has been little explored but is already present and future in the tourism sector. In this study, it is intended to classify and find the main motivations of tourists to take this type of vacation and see the relationship of this trend with today's environmental awareness. To meet the objectives, a systematic literature review is carried out from which the 3 main motivations that drive this type of disconnection tourism are constructed: nature and authenticity, reconnecting with oneself, and the search for human connection.

This paper examines women's power relative to that of their husbands in 23 Sub-Saharan African countries to determine how it affects women's health, reproductive outcomes, children's health, and children's education. The analysis uses a novel measure of women's empowerment that is closely linked to classical theories of power, built from spouses' often-conflicting reports of intrahousehold decision making. It finds that women's power substantially matters for health and various family and reproductive outcomes. Women taking power is also better for children's outcomes, in particular for girls' health, but it is worse for emotional violence. The results show the conceptual and analytical value of intrahousehold contention over decision making and expand the breadth of evidence on the importance of women's power for economic development
